Elevated bradykinin stimulates sensory nerves in the respiratory tract, leading to a characteristic dry, persistent cough. This cough is a well-documented and common adverse effect of ACE inhibitors like lisinopril, often necessitating medication discontinuation. Unlike other drugs, this cough is not due to allergy or infection but is a direct pharmacologic effect related to bradykinin accumulation.
